At Sinai Health, we care, create possibilities and offer hope. We are looking for an experienced Nurse Practitioner to support our Medical/Neuro Complex Care and Reconditioning Service department. 

Job Outline:

  • Practices within the full scope of the Nurse Practitioner role conducting assessments, formulating diagnoses and developing comprehensive plans of care.
  • Provides assessment, intervention, monitoring and follow-up for a broad range of acute and chronic conditions.
  • Integrates best practices and evidence into clinical practice; provides exemplary quality care through the application of advanced clinical and theoretical knowledge and skills.
  • Demonstrates sound clinical reasoning in decision making, planning and implementing care.
  • Collaborates with members of the Interprofessional Team in coordinating and providing care for patients.
  • Utilizes skilled communication with patients, families, healthcare providers, and partners, to support coordinated, compassionate care, including Advance Care Planning and goals of care conversations.
  • Uses effective patient/family counseling skills.
  • Employs excellent organizational, critical appraisal and analytical skills.
  • Builds partnerships with community and external stakeholders to support care planning and continuity of care.
  • Continuously identifies areas of improvement and provides leadership for quality improvement initiatives.
  • Supports education and development of the interprofessional team.
  • Provides leadership and promotes excellence in patient-centred nursing care through expert practice, education, consultation and research.

Education & Qualifications:

  • Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP-Adult or NP- Primary Health Care required.
  • Master's in Nursing/Nurse Practitioner (MN-NP) or Master's in Nursing with successful completion of a post Master's Nurse Practitioner Certificate Program from a recognized school of graduate studies required.
  • 5 years of relevant acute care or post-acute experience in the inpatient setting (e.g. medicine, medical surgical nursing, intensive care, complex continuing care).
  • 1-3 years of primary care experience an asset.
  • Experience with complex wound care assessment and treatment an asset.
  • Additional specialty geriatric education and experience with seniors and frail elderly preferred.
  • Experience in caring for people with mental health and/or geriatric psychological conditions an asset.
  • Current CPR certification.
  • Demonstrated experience through roles of advanced practitioner, educator, researcher and leader.
  • Demonstrated strong communication and interpersonal skills.
  • Highly developed analytical, problem solving and critical thinking skills.
  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology.
