As a Registered Practical Nurse in Medical Rehabilitative Care, you'll support patients recovering from complex medical conditions as they work toward improved strength, mobility, and independence. You’ll deliver consistent, practical nursing care that helps patients achieve their day-to-day recovery goals, while observing and responding to changes in their health status. About the Medical Rehabilitative Unit (4 North):
At Hennick Bridgepoint Hospital, our 4N Medical Rehabilitative Care team supports patients through some of their most challenging and transformative recoveries. Our team of over 45 individuals, brings together nurses, physicians, and professionals from other health disciplines to support complex recoveries. We care for patients with a wide range of medical conditions (such as infections, recent surgeries, deconditioning, frailty, and cognitive impairment) who are working to regain strength, mobility, and functional independence. Many patients have multiple diagnoses and require both medical oversight and rehabilitation-focused support to progress with daily activities. Our team is highly collaborative and focused on helping each patient reach meaningful recovery goals.

In this role you will:

  • Practice independently and accountably within the RPN scope of practice, adhering to CNO standards and Sinai Health policies
  • Identify changing patient needs to initiate timely consultation and collaboration with Registered Nurses and the wider interprofessional team
  • Monitor and document the progress of clinical care plans and outcomes, adjusting strategies as needed to optimize treatment goals and patient safety
  • Participate in the mentoring of learners, peers and other regulated and unregulated health professionals
  • Provide health education to patients and their families using effective teaching strategies
  • Assess spiritual, cultural and psychosocial needs of patients
  • Provide comprehensive and accurate documentation of assessments, care provision, and evaluations
  • Collaborate with the patient, significant others and multidisciplinary team in the provision of care
  • Provide comprehensive and accurate documentation of assessments, care provision, evaluations, discharge and transition planning
  • Perform other duties consistent with the job classification as required


Job Requirements:
Mandatory
  • Graduate of a CNO-approved Practical Nursing (PN) program 
  • Currently registered as an RPN and in good standing with the College of Nurses of Ontario (CNO)
  • Current Basic Cardiac Life Support (BCLS) certification

Preferred
  • Experience with electronic health record (EHR) and clinical documentation

  • Effective time-management and organizational skills
  • Excellent problem solving and critical thinking skills
  • Strong collaboration skills to build professional relationships and support integration with health care team
  • Computer proficiency and/or basic to intermediate Microsoft Office (Word and Outlook) skills
  • Communication skills including active listening, conflict resolution and relational practice
  • Demonstrated leadership skills
  • Demonstrated satisfactory job performance and attendance history required

Rotation: Night

Salary: Minimum - $34.812/hour; Maximum - $35.970/hour
